           The package of services for genetic parents includes :

The price-list of the prices under the surrogate motherhood program. 

• 2850 EUR - to be paid under the Contract to be signed with the Center for selection of a surrogate mother from the database and her preparation for the Program (the services to be provided under this Contract are as follows: comprehensive medical examination of a surrogate mother at the Clinic – Human Reproduction Center, preparation of the documents such as the psychologist opinion, no criminal record certificate, statement of the surrogate mother’s husband authorizing her participation in the surrogate motherhood program, etc., legal support of the program till the date of conclusion of the contract between the surrogate mother and genetic parents (Customers);
• 6000 EUR - to be paid as the compensation due to the surrogate mother for the whole period of pregnancy (10 % of this amount shall be paid at the time of confirmation of the fact of pregnancy, while the remaining 90 % shall be paid 10 days before the expected birth date);
• 1000 EUR - bonus to be paid to a surrogate mother; ;
• 250 EUR X 9=2250 EUR - to be paid monthly as the allowance (the so called “food money”) due to the surrogate mother during 9 months of her pregnancy;
• 1000 EUR - cost of maternity clothes for a surrogate mother for the whole pregnancy period;
• 2526 EUR - one attempt of IVF or ICSI procedure (if a genetic mother makes use of her own genetic matter), the ovulation stimulation madicines should be paid for separately;
• 4939 EUR - one attempt of procedure IVF, cost of service of the donor Ovum and full cost of services of clinic, including all necessary medicines for stimulation of the ovum donor and preparation of surrogate mother's endometrial;
• 4090 EUR - medical supervision during pregnancy - cost of services of clinic and all necessary analyses; cost of sorts in clinic; - medic cines during pregnancy (a standard package of medicines, unforeseen circumstances are not taken into account(discounted)); Since 3 weeks of pregnancy till a birth of the child, including childbirth.
• 400 EUR - to be paid for execution of the insurance policy to insure the life of the surrogate mother during the delivery;
• 1200 EUR - to be paid for legal execution of essential registration documents, including the child birth certificate, traveling and other documents to be submitted to the embassy by the genetic parents in person;
• 700 EUR - cost of indemnification of the donor;
• 700 EUR - cost of services of the center including medical inspection and support up to a puncture Ovum donor;
• 450 EUR - Cost of service of the curator - support and the control over a course of pregnancy during 9 months, (full or partial):
• The partial control - hourly visiting of substitute mother, the control over performance of treaty obligations, a feed(meal), support to the doctor, cost - 12,5 EUR for one business hour;
• The full control is a round-the-clock residing together with substitute mum, performance of all homework, support to the doctor and the full report on the done job, cost – 25 EUR in day, (Granting of separate habitation);
• 150 EUR - Cost of a demountable apartment a month.

  Medical services to be paid for separately:  

• about 2526 EUR - one attempt of IVF or ICSI procedure (if a genetic mother makes use of her own genetic matter), the ovulation stimulation madicines should be paid for separately;
• about 4939 EUR - to be paid under the IVF program contract, covering the following services: one attempt of the IVF or ICSI procedure, ovum donor services and full clinic services, including all necessary medicines used for stimulation of the ovum donor and preparation of surrogate mother's endometrium;
• about 4090 EUR - to be paid under the contract for medical maintenance and supervision during the whole pregnancy period: the cost of the clinic services and all necessary medical tests, cost of the delivery at the maternity hospital, medicines to be used in the course of pregnancy (standard package of medicines, excluding force-majeure circumstances).

  "Economical" package deal – 2850EUR:
Presentation of candidature of surrogate mother from database of surrogate motherhood center, including the medical inspection of surrogate mother stipulated by the medical law of Ukraine

• Psychological diagnostics of potential surrogate mother as to her psychological fitness and readiness for participation in the surrogacy motherhood program;
• Complex medical examination of potential surrogate mother to confirm her fitness for extracorporeal fertilization, pregnancy and birth of a healthy child;
• Primary medical advice, preparation of contract between genetic parents and surrogate mother with consideration of all possible aspects of individual situation of customers and introduction of the necessary clauses to the child-bearing contract;
• Recommendations on choice of reproduction medicine clinic and other required medical institutions;
• Services of translator during primary advice, and preparation of contract between genetic parents, and surrogate mother;
• Coordination and the control of the surrogacy program, over the conclusion of the contract between genetic parents, and surrogate mother;
• The IVF procedure and the period of medical pregnancy maintenance shall be paid for separately and shall not be a part of the package cost.

• 2850 Евро - в стоимость услуг центра, входит - подбор кандидатуры суррогатной матери, предоставление базы данных суррогатных матерей, заключение психолога, комплексное медицинское обследование суррогатной матери в клинике репродуктивной медицины, справка о не судимости суррогатной матери, согласие супруга на участие в программе его жены - суррогатной матери, юридическое сопровождение программы до заключения договора между суррогатной матерью и генетическими родителями (Заказчиками), услуги переводчика (если необходимо), гостевые услуги (стоимость гостиницы и транспорта оплачивается дополнительно);
• 6000 Евро - компенсация суррогатной маме за весь срок беременности (оплата поэтапная -10% по факту наступления беременности, 90% за 4 - 6 недель до рождения ребенка- денежные средства перечисляются на расчетный счет суррогатной матери;
• 1000 Евро - премиальная выплата для суррогатной мамы;
• 250 Евро X 9 = 2250 Евро - ежемесячное содержание на 9 месяцев беременности – выплачивается помесячно;
• 1000 Евро - стоимость одежды для суррогатной мамы за весь период беременности;
• 2526 Евро - одна попытка процедуры ЭКО или ИКСИ (если генетическая мама использует собственный генетический материал), препараты для стимуляции овуляции оплачиваются дополнительно;
• 4939 Евро - одна попытка процедуры ЭКО или ИКСИ,с участием донора яйцеклетки и полная стоимость услуг клиники, включая все необходимые медикаменты для стимуляции донора яйцеклетки ;
• 4090 Евро - медицинское наблюдение во время беременности – стоимость услуг клиники и всех необходимых анализов, стоимость родов в клинике, медикаменты во время беременности (стандартный пакет медикаментов, форс. мажорные обстоятельства не учтены);
• 400 Евро - медицинское страхование жизни  суррогатной мамы  во время родорозрешения и послеродовой период;
• 1200 Евро - оформление документов на ребенка, включая свидетельство о рождении ребенка, для дальнейшей подачи  документов в посольство;
• 700Евро- стоимость компенсации донора;
• 700Евро - предоставление кандидатуры донора яйцеклетки, стоимость услуг Центра включая медицинское обследование и сопровождение донора до пункции яйцеклетки;
• 450 Евро – стоимость  услуги куратора;
• 12,5 Евро - за один час работы в неделю - частичный контроль - почасовое посещение суррогатной матери, контроль за выполнением договорных обязательств, питания, сопровождение к врачу;
• 25 Евро в сутки - полный контроль - это круглосуточное проживание вместе с суррогатной мамой, выполнение всей домашней работы, сопровождение к врачу и полный отчет по проделанной работе (предоставление отдельной квартиры);
• 150 Евро - стоимость съемной квартиры в месяц. (при необходимости по индивидуальному желанию суррогатной матери);

«Экономный» пакет услуг - 2850 Евро

• Предоставление кандидатуры суррогатной матери из базы данных Центра Суррогатного материнства "La Vita Felice";
• Психологическую диагностику потенциальной суррогатной матери на предмет психологической пригодности и готовности суррогатной матери к участию в программе суррогатного материнства;
• Комплексное медицинское обследование потенциальной суррогатной матери для подтверждения пригодности к экстракорпоральному оплодотворению, вынашиванию беременности и рождению здорового ребенка;
• Первичная юридическая консультация, составление договора между генетическими родителями и суррогатной матерью с учетом всех возможных нюансов индивидуальной ситуации заказчиков и внесением необходимых пунктов в договор «о вынашивании ребенка»;
• Рекомендации по выбору клиники репродуктивной медицины, других необходимых медицинских учреждений;
• Услуги переводчика в процессе первичного консультирования и составления договора между генетическими родителями и суррогатной матерью;
• Координация и контроль программы, до заключения договора между генетическими родителями и суррогатной матерью;
• Стоимость медицинских услуг оплачивается дополнительно.
